For your convenience, we accept cash, personal checks, money orders and major credit cards including, VISA, MasterCard, & DISCOVER.  Payment is expected at the time services are performed. When more extensive dental care is necessary, our office kindly accepts Visa, MasterCard, Discover and Care Credit.

Insurance

Please contact our dental office if you have any questions regarding your insurance coverage.

We will make every attempt to file your dental insurance for you if applicable. You are responsible for providing our office with all of the needed information requested. Please understand we will do everything thing we can to collect your benefits, however it is ultimately the patients responsibility.

Please Note:You authorize all payment of your dental benefits to Dr. Amber Reid. You agree to be responsible for all charges for dental services and materials not covered by your dental plan.
